I'm a big fan of RP 90's and I keep mine at 65% and haven't had a problem, is it possible the B&M you purchased them at kept them at a different RH, not sure but could be a problem..

I find with the 90s and 92s, the cap does seem to come loose more times than not. Once that happens, the wrapper is so thin, it starts it's unraveling process. Cutting does this and even punching will loosen the cap. I've been through many of these cigars and I'd say they unravel in higher percentages than any other brand I can think of.
