Around here, the Costco packaging is shrink wrapped flats. For those, once you've taken the sodas off, flip the cardboard over and it should be underneath. If you look under the flat before you open it you'll see where they put it, there's a blacked out rectangle with arrows pointing to it on the shrink wrap.

The cardboard box type packaging, as G said, it should be on the inside flap

I'm not sure about the shrink wrapped bottle packs. I think those might be under the caps, with the exception of water and the minute maid juices, which is somewhere on the cardboard bottom piece I believe.

It'll be a 12 or 14 digit code, dot matrix printed, and might say something like "My Coke Rewards Code" near it.
