The one I'm using is called
Food And Exercise Diary. It's pretty full featured and works well for me, the trick is being disciplined about putting in the information about what you eat and how much you exercise (and optionally things like body fat %, and body measurements).
I find I do way better if I'm using something like this that lets me chart overall progress, and see at a glance where I sit for any given day.
